A no-show appointment policy is a short written rule that tells clients what counts as a missed appointment, how much notice they must give to cancel, and what happens if they do not show up. A fair, workable policy includes:

  1. Definitions: what counts as a no-show, a late cancellation, and a late arrival
  2. A notice window, usually 24 or 48 hours
  3. Consequences, such as a fee, a deposit kept, or limits on future booking
  4. Exceptions for emergencies, with a way to ask for one
  5. How you will tell people: at booking, in reminders, and on a form they sign

Healthcare practices must also follow payer rules: Medicare allows a missed appointment fee only if the same policy applies to all patients, and Medicaid patients cannot be billed for missed appointments. The templates below cover medical, dental, therapy, salon, and service businesses.

What Counts as a No-Show

Most disputes start because nobody defined the terms. Spell them out.

  • No-show (no call, no show): the client misses the appointment and does not contact you before it starts.
  • Late cancellation: the client cancels inside your notice window, for example less than 24 hours before.
  • Late arrival: the client arrives so late that the visit cannot go ahead in full. Set a clear limit, such as 15 minutes.

NYU Langone’s Family Health Centers dental clinics, for example, treat arriving more than 15 minutes late as late, and not arriving within three hours of the appointment time as a no-show.

How to Create a No-Show Appointment Policy

  1. Check your no-show numbers first. Measure your rate by service, provider, and day. A policy for a 2% problem should be lighter than one for a 20% problem.
  2. Pick a notice window. 24 hours suits short visits; 48 hours suits long or high-value appointments. Salon software provider Zenoti suggests 24 to 48 hours as typical for salons.
  3. Decide the consequence. A fee, keeping a deposit, requiring a card on file, or limiting future advance booking.
  4. Set a grace rule. Many businesses waive the first no-show and apply the policy from the second.
  5. Write the exceptions. Emergencies, illness, weather, and similar events, plus how a client can ask for an exception.
  6. Check the rules for your industry. Healthcare has payer rules; some states and cities regulate fees or deposits.
  7. Tell clients before it matters. Show it at booking, include it in confirmations and reminders, and have new clients sign it.
  8. Apply it the same way to everyone, and keep a record of each missed appointment.

Rules to Know Before Charging No-Show Fees

This is general information, not legal advice.

  • Medicare: CMS allows practices to charge Medicare patients for missed appointments only if the same policy and the same amount apply to all patients. Medicare itself does not pay for missed appointments, so the fee is billed to the patient, not to Medicare.
  • Medicaid: CMS guidance does not allow providers to bill Medicaid patients for missed appointments.
  • Medical ethics: The AMA’s ethics guidance, as reported in 2024 coverage of no-show fees, says it is ethical for physicians to charge for missed appointments when patients are fully advised in advance.
  • How common fees are: MGMA’s January 2025 poll found 42% of medical groups charged a no-show fee, and practices with a fee reported improvement more often than those without (25% vs 16%), though the data is self-reported.
  • Dental documentation: The ADA’s CDT codes D9986 (missed appointment) and D9987 (cancelled appointment) let dental offices record missed visits, whether or not they charge.

Real-World Examples

NYU Langone Family Health Centers (dental)

NYU Langone dental appointment policy and Zocdoc booking platform comparison, showing appointment confirmation, no-show restrictions, same-day booking access, and patient scheduling options.

The NYU Langone Family Health Centers dental policy asks patients to confirm at least two business days before the visit, or the appointment is canceled and rescheduled. Patients with two or more no-shows in 12 months can no longer book dental appointments in advance and must use same-day slots. Patients can ask for an excused no-show for emergencies. The policy page does not mention a fee.

This is a good model for practices that serve patients who cannot easily pay fees: it protects the schedule by changing booking access instead of charging money.

Zocdoc

Zocdoc is a booking marketplace, not a practice, so it does not set a single cancellation deadline for patients. Its own guidance says patients who repeatedly miss, reschedule, or cancel may have their access limited, suspended or terminated under its community standards. Each practice’s own no-show policy still applies to the visit itself.

How to Communicate and Enforce Your Policy

  • At booking: show the policy on your online booking page and read the short version on the phone.
  • In writing: have new clients sign it, in person or digitally.
  • In every reminder: a single line, such as “Please give 24 hours’ notice to change your appointment.”
  • After a no-show: call the same day to rebook, then send the letter above.
  • Keep records: log every no-show and late cancellation, and any exception you granted.
  • Train staff: everyone should explain the policy the same way and know who can approve exceptions.

Mention the policy in reminders in general terms only. For healthcare, keep balances and fees out of texts, as our guide to HIPAA-compliant appointment reminders explains.

A Policy Works Best With Reminders

A policy deters no-shows after the fact. Reminders stop them from happening. The strongest setup combines both:

  • A confirmation request 1 to 3 days before, so you know who is coming. Our appointment confirmation call script shows what to say.
  • A short reminder on the day, with the policy line.
  • A call to anyone who does not confirm, especially clients with past no-shows.
  • A waitlist to fill cancelled slots.

For ready-made reminder wording, see our appointment reminder templates. Medical and dental offices can find specialty timing in our patient appointment reminders and dental appointment reminder guides.

How The DBT AI Helps Reduce No-Shows

The DBT AI makes appointment reminder calls so fewer appointments reach the point where your policy applies.

FAQ

How do you create a no-show appointment policy?

Measure your current no-show rate, choose a notice window (24 or 48 hours), decide the consequence (fee, deposit or booking limits), add a grace rule and exceptions, check industry and payer rules, and share the policy at booking, in reminders and on a signed form.

What is a no-call, no-show appointment policy?

It covers clients who miss an appointment without contacting you beforehand. It usually sets a fee or a booking restriction and explains how the client can rebook or ask for an exception.

Can a doctor charge for a no-show?

Yes, in many cases, if patients were told in advance. Medicare patients can be charged only if the same fee applies to all patients, and Medicaid patients cannot be billed for missed appointments.

How much should a no-show fee be?

It depends on your service. Medical offices often charge a flat fee, while salons commonly charge 50% to 100% of the booked service. Choose an amount that covers part of your lost time without driving clients away, and apply it consistently.

What is NYU Langone’s no-show policy?

For its Family Health Centers dental clinics, NYU Langone asks patients to confirm two business days ahead. Two or more no-shows in 12 months mean patients can only book same-day appointments. Excused no-shows can be requested for emergencies. Other NYU Langone services may have their own policies.

What is Zocdoc’s missed appointment policy?

Zocdoc does not set one cancellation deadline for patients. Its guidance says patients who repeatedly miss, reschedule or cancel appointments may have their Zocdoc access limited, suspended, or terminated. Each practice’s own policy also applies.

What should a salon no-show policy include?

A notice window (usually 24 to 48 hours), a fee for late cancellations and no-shows, a late arrival limit, a card on file or deposit requirement, and a rule for repeat no-shows, such as prepaying future bookings.

Should the first no-show be forgiven?

Many businesses waive the first one and apply the policy from the second. It builds goodwill, and clients who know about the policy usually do not miss again.
