Patient appointment reminders are calls, texts, emails or letters that tell patients about an upcoming visit and give them an easy way to confirm, cancel or reschedule. They work best as a short sequence:

  • A confirmation right after booking
  • A reminder a few days before that asks for a reply
  • A phone call to anyone who does not reply, especially patients with past no-shows or visits booked weeks ahead
  • A short message on the day
  • A same-day call after a no-show to rebook

Research supports this mix. A review of 105 studies in Health Policy put the average no-show rate at about 23%, with long booking lead times and prior no-shows as the strongest warning signs. Reaching the patient in person matters most: in a VA primary care study, patients who were spoken to directly missed 3% of visits, against 24% for those who only got a voicemail. Most practices now automate the routine messages and use appointment reminder calls for the patients most likely to miss.

How Often Patients Miss Appointments

No-show rates vary widely by setting, which is why a single “average” can mislead. Here is what published research and surveys show.

SettingWhat was measuredFindingSource

Outpatient imaging2.9 million visits, 2000 to 20153.25% overall; 6.99% for mammography; 9.46% when booked six months outJACR, 2018

Three patterns run through the data:

  • Lead time matters. The longer the gap between booking and visit, the more likely a patient is to miss it. The imaging study saw no-shows rise from 2.71% at one week to 9.46% at six months.
  • Past behavior predicts future behavior. Prior no-shows were among the most consistent predictors in the Health Policy review.
  • Mental health can run much higher. One university psychiatry clinic measured 31%, roughly ten times the imaging rate, which is why reminders and follow-up matter even more in behavioral health.

Why Patients Don’t Show Up

Forgetting is the most common explanation, but it is not the only one, and patients and providers often disagree on the cause.

  • Providers blame forgetting. In a 2026 Tebra survey reported by MGMA, 81% of providers named forgetting as the main cause.
  • Patients point to life. The same survey found patients more often cited work conflicts (31%), weather (30%) and emergencies (27%). 69% wanted to reschedule online without calling.
  • Emotions play a part. In a study in the Annals of Family Medicine, 65% of patients who missed visits described emotional barriers such as fear of bad news, and many skipped once their symptoms improved. 44% mentioned feeling disrespected by long waits.
  • Many patients do not know the rules. 41% in the same study did not understand the clinic’s scheduling or no-show policy.
  • Costs and transport. MGMA’s 2026 poll comments mention patient costs, gas prices, transportation and work conflicts as drivers of rising no-shows.

The takeaway: a reminder fixes forgetting. Making it easy to reschedule, explaining the policy and calling high-risk patients fixes much of the rest.

How to Remind Patients of Appointments: A Risk-Based Sequence

Sending the same three texts to everyone is common. Matching effort to risk works better.

How to Remind Patients of Appointments: A Risk-Based Sequence

Every patient

  • At booking: confirmation by text or email with date, time, location and how to change it
  • 3 to 7 days before: reminder with forms, prep instructions and a reschedule link
  • 1 to 2 days before: confirmation request (reply C or R)
  • Morning of: short reminder with arrival details

Higher-risk patients

Add a live or automated phone call 1 to 2 days before for:

  • Patients with one or more past no-shows
  • Visits booked more than a month ahead
  • New patients
  • Behavioral health, specialty and procedure visits where a miss is costly
  • Patients who did not reply to the text

After a no-show

  • Call the same day to check in and rebook
  • Text a rebooking link if the call goes unanswered
  • Flag the record so the next visit gets the higher-risk sequence

When are reminder calls usually placed? Most practices call 1 to 3 days before the visit, during daytime hours, so there is still time to fill the slot if the patient cancels. For call wording, see our appointment confirmation call script.

Choosing the Channel: Call, Text, Email or Letter

  • Text is fast, cheap and easy to reply to. A Cochrane review found text reminders raised attendance from 67.8% with no reminder to 78.6%.
  • Phone calls performed about the same as texts in that review (80.3%), but they reach patients who ignore texts and let staff solve problems on the spot. Calls also suit older patients.
  • Email carries forms, prep instructions and directions well, but is easy to miss on its own.
  • Letters and postcards still help with recall for older patients and for visits booked months out.

Most practices use text for everyone and calls for the higher-risk group. For ready-made wording across channels, use our appointment reminder templates.

Patient Appointment Reminders by Specialty

Patient Appointment Reminders by Specialty

Primary care and family practice

Annual physicals and chronic care follow-ups are booked far ahead, so add a reminder 1 to 2 weeks before and a recall message when patients are overdue.

Mental health and therapy practices

Automated appointment reminders for therapy practices should be warm and low pressure. Keep the practice name neutral if it reveals the service, offer an easy reschedule and follow up quickly after a missed session.

Physical therapy and chiropractic

These patients have many short, recurring visits. Remind before each visit and send a weekly schedule summary so patients do not drop off mid-plan.

Imaging and radiology

Prep instructions (fasting, clothing, contrast) matter as much as the time. Visits booked months ahead, especially screening mammograms, need extra reminders.

Pediatrics

Remind the parent or guardian, not the child, and confirm who will attend. In a pediatric dental trial in JADA, voice reminders to families led to fewer missed visits than text alone (8.2% vs 17.7%). Our dental appointment reminder guide covers dental offices in detail.

Veterinary clinics

Vet practices remind pet owners about visits and also about vaccines and annual checkups. Many use reminder tools that connect to their practice management system. PetDesk, for example, sends reminders two days before visits by email, text, push notification or postcard and lists integrations with systems such as Cornerstone, AVImark, ezyVet and Covetrus Pulse.

Patient Appointment Reminder Software and Systems

Most practices choose from three types of tools.

Reminders built into the EHR or practice management system

Many EHR and practice management systems include reminders. SimplePractice, used widely by therapists, says it sends text, voice and email reminders and lists them as free. NextGen Healthcare lists appointment reminders, an automated waitlist and self-scheduling in its patient experience platform. Start here, because these tools already know your schedule.

Patient engagement platforms

Patient engagement platforms typically add two-way texting, recall campaigns and online booking on top of your EHR. Solutionreach, which started out as Smile Reminder, is one long-running example. These platforms suit groups that want one place for all patient messaging.

AI voice reminder systems

Automated voice appointment reminder systems in healthcare now use conversational AI instead of “press 1” recordings. The AI calls, speaks naturally, confirms or cancels, updates the schedule and texts the details. This suits practices with many non-responders, older patients or not enough staff to make confirmation calls.

What to check before you choose:

  • Two-way sync with your EHR or scheduling system
  • Two-way replies (confirm, cancel, reschedule)
  • Calls as well as texts and email
  • Different sequences for different visit types and risk levels
  • A signed business associate agreement and control over message content
  • Reports on confirmation rate and no-show rate by provider and visit type

On the HIPAA side, appointment reminders are allowed without a separate authorization. Our guide to HIPAA compliant appointment reminders covers what to include and what to leave out.

Doctor Appointment Reminder Templates

Multichannel Healthcare Appointment Reminders

Text, two days before:

[Practice]: Reminder: appointment with [Provider] on [Day] at [Time]. Reply C to confirm or R to reschedule. STOP to opt out.

Doctor appointment reminder letter (for visits booked months ahead):

Dear [Patient name],

This letter is a reminder of your appointment with [Provider] at [Practice] on [Day, Date] at [Time].

Please arrive 15 minutes early and bring your photo ID, insurance card and a list of current medications. If you need to change your appointment, please call us at [Number] at least [24/48] hours before.

Sincerely, [Practice]

No-Show Policies and Fees

A clear policy, shared at booking and in reminders, reduces confusion and disputes.

  • Medicare: CMS allows practices to charge Medicare patients for missed appointments only if the same policy and the same amount apply to all patients. Medicare itself does not pay for missed appointments.
  • Medicaid: CMS guidance does not allow providers to bill Medicaid patients for missed appointments.
  • Fees are common: MGMA’s January 2025 poll found 42% of practices used a no-show fee. Practices with a fee reported improvement more often, though the data is self-reported.
  • Explain it early: state the notice window (for example, 24 hours) and what happens after a no-show, in writing and in reminders.

What happens if you no-show a doctor’s appointment? Usually a call or message to rebook, sometimes a fee under the practice’s written policy, and in some practices a requirement to confirm by phone before future visits.

Using Data to Reduce No-Shows

Practices that track a few numbers improve faster than those that only send more messages.

  • No-show rate by provider, visit type, day, and lead time
  • Confirmation rate before the cut-off
  • No-show rate for confirmed vs unconfirmed appointments
  • Rebooking rate within 30 days of a no-show or cancellation
  • Slots refilled from waitlists

Use the results to target calls at the highest-risk groups, shorten lead times where you can, and offer same-day slots from a waitlist. Our post on how AI appointment setters reduce no-shows covers rebooking workflows.

How The DBT AI Handles Patient Reminders

The DBT AI makes reminder, recall, and rebooking calls for medical practices as part of its virtual receptionist for medical practices.

  • Reminder call and text before each visit, with date, time, and provider
  • Patients confirm or cancel on the call, whoever booked the appointment
  • Separate rebooking calls for missed visits (patients cannot change the time on the reminder call itself)
  • Recall calls to patients overdue for a physical or follow-up, including lapsed patients
  • Works with your EHR or calendar; our medical page lists systems such as Epic, athenahealth, and eClinicalWorks, so confirm yours on the demo
  • HIPAA-ready, with Spanish and other languages; ask about a business associate agreement on the demo
  • Set up for you, with a one-time setup fee and a per-minute rate

FAQ

What platforms offer automated voice appointment reminders for clinics?

Some EHR and practice systems, such as SimplePractice, include voice reminders. Patient engagement platforms and AI voice services, such as The DBT AI, place reminder calls that let patients confirm or cancel and update the schedule. Check EHR sync, two-way replies and a signed BAA before choosing.

What are the best practices for reducing patient no-shows in medical settings?

Send a reminder sequence with a confirmation request, call higher-risk patients, shorten lead times where possible, make rescheduling easy, share a written no-show policy and call every no-show the same day.

How do you remind patients of appointments?

Send a confirmation at booking, a reminder a few days before that asks for a reply, a call to anyone who does not respond, and a short message on the day. Use text for most patients and calls for those most likely to miss.

What are best practices for automating patient appointment reminders in healthcare?

Connect reminders to your EHR, set different sequences by visit type and risk, use two-way replies, keep messages free of clinical details, honor opt-outs and track no-show and confirmation rates by provider.

Is there HIPAA-compliant conversational AI for healthcare appointment reminders?

Yes. Conversational AI can send HIPAA compliant reminders if the vendor signs a business associate agreement, protects call data and lets you control what each call says. The FCC treats AI voices as artificial voices, so consent rules for automated calls also apply.

How do you reduce no-show appointments at a vet clinic?

Remind owners two days before by their preferred channel, include pre-visit instructions, send vaccine and checkup reminders, offer easy confirm and cancel links, and call owners who do not respond or who have missed before.

Why do patients no-show for appointments?

Forgetting is common, but patients also cite work conflicts, weather, emergencies, costs, transportation, fear of bad news and not understanding the clinic’s policy. Long booking lead times and past no-shows are strong predictors.

What appointment reminder tools work with veterinary practice management systems?

Tools such as PetDesk connect to systems including Cornerstone, AVImark, ezyVet, Covetrus Pulse and others, and send reminders by email, text, push notification or postcard. Check that the tool supports your specific system before signing.

What is the average no-show rate for doctor appointments?

A review of 105 studies put the average at about 23%, but rates vary widely by setting, from around 3% for outpatient imaging in one large study to over 30% in some behavioral health clinics.

What is the average no-show rate for mental health appointments?

It varies by clinic, but behavioral health tends to run higher than most specialties. One university psychiatry clinic reported a 31% baseline no-show rate before introducing reduction strategies.

Do you get charged for a no-show doctor appointment?

Only if the practice has a no-show fee policy. Medicare patients can be charged only if the same fee applies to all patients, and Medicaid patients cannot be billed for missed appointments under CMS guidance.

Can Medicaid patients be billed for no-show appointments?

No. CMS guidance does not allow providers to bill Medicaid patients for missed appointments.

Are healthcare appointment reminders considered marketing?

No. HHS treats appointment reminders as part of treatment, so they are not marketing and do not need a separate authorization, as long as they are only about the appointment.

Does HubSpot send automatic appointment reminders to patients?

HubSpot can send automated reminders, but patient health information is allowed only on Enterprise plans with its sensitive data features and BAA, and only in the features HubSpot lists as covered. Most practices use their EHR or a healthcare-specific tool instead.

Start With Your Highest-Risk Appointments

You do not need to rebuild everything at once. This week, pull a list of patients with a past no-show or a visit booked more than a month ahead, and add a reminder call for them. Track the no-show rate for that group for a month and compare.

If you want those calls made and logged for you, talk to The DBT AI team and hear a patient reminder call on your own phone.
